Emergency Visits Seen Increasing With New Health Law

 “Supporters of President Obama’s health care law had predicted that expanding insurance coverage for the poor would reduce costly emergency room visits because people would go to primary care doctors instead. But a rigorous new experiment in Oregon has raised questions about that assumption, finding that newly insured people actually went to the emergency room a good deal more often.

The study, published in the journal Science, compared thousands of low-income people in the Portland area who were randomly selected in a 2008 lottery to get Medicaid coverage with people who entered the lottery but remained uninsured. Those who gained coverage made 40 percent more visits to the emergency room than their uninsured counterparts during their first 18 months with insurance.

The pattern was so strong that it held true across most demographic groups, times of day and types of visits, including those for conditions that were treatable in primary care settings.

The findings cast doubt on the hope that expanded insurance coverage will help rein in emergency room costs just as more than two million people are gaining coverage under the Affordable Care Act. And they go against one of the central arguments of the law’s supporters, that extending insurance to large numbers of Americans would reduce emergency room use, and eventually save money.

In remarks in New Mexico in 2009, Mr. Obama said: “I think that it’s very important that we provide coverage for all people because if everybody’s got coverage, then they’re not going to the emergency room for treatment.”

The study suggests that the surge in the numbers of insured people may put even greater pressure on emergency rooms, at least in the short term.”

President Obama announces Hardship Exemption

January 5th

On December 19th, President Obama announced that: 

Anyone who can show that their individual/family policy has been cancelled, they will be eligible to buy a “catastrophic” plan through the Nevada Health Link and qualify for a “hardship exemption”.  This means they would not be fined if they can’t, or don’t, enroll in a plan this year.

These catastrophic plans, however, cannot get a subsidy.

ICD-10 Medical Billing Changes Coming


Many changes to the way medical billing works will be coming on Oct. 1, 2014.  The whole health care medical billing system will change over to a new computerized coding system used by doctors and hospitals and the entire medical community.

The ICD-10 will be used to describe what medical conditions a patient may have and how much the insurers should pay for specific treatments.  The new ICD-10 system is a very precise and detailed medical billing coding system, more so than the current existing ICD-9.

There is a push back from many physicians and providers claiming that their computer systems and employees won’t be ready in time.

In October of this year, when you figure your Household Income for an Obamacare plan, please note:
Household Income– Everyone in the household’s income is factored into the health insurance premium. So if you have a 21 year old adult child that is going to be on their parents plan, you must include their income as part of the “Household income” whether they live at home or not, to figure out the Advanced Premium Tax Credit. Read below: “All other individuals who were taken into account in determining the taxpayers family size.” (See below) Remember this, it is important, because at the end of the year, when you settle up with the IRS while doing your taxes, your “Advanced Premium Tax Credits” you received cannot be greater than your allowances, or you will owe the IRS money back. 

Household Income is defined in 26 US Code 36B(d)(2)(A):
The term “household income” means, with respect to any taxpayer, an amount equal to the sum of
(i) the modified adjusted gross income of the taxpayer, plus 
(ii) the aggregate modified adjusted gross incomes of all other individuals who— 
(I) were taken into account in determining the taxpayer’s family size under paragraph (1), and
(II) were required to file a return of tax imposed by section 1 for the taxable year.

New Health Care Terms…..Income Changes (Redetermination)– If your income goes up or goes down, it’s probably a very good idea to go back to the “Exchange” (online website) and put in your new income. This way you can have your Subsidy adjusted to either get less “Subsidy” or get more “Subsidy” based on your new income. You don’t want to have to owe the Government money back at the end of the year because you received too much Subsidy, and if your income went down, you’ll probably be eligible for more subsidy. BE VERY CAREFUL NOT TO GET TOO MUCH SUBSIDY. The reason being would be you’d have to pay the extra back. For example: If your health insurance premiums are $1,000/month and you get a Subsidy of $800/month and you only have to pay $200/month. At the end of the year when you have to settle up your taxes on April 15th of each year, the Government will check your income, and IF you were only supposed to have received an Advanced Tax Credit of $700/month instead of $800/month, you’ll owe the Government an extra $100/month X 12 months is $1,200.
